Ubuntu Clinical Outreach Project

Executive Summary

The Ubuntu Clinical Outreach Project, implemented by the Durban Heart Rhythm Association (DHRA), aims to enhance access to specialised cardiac care among underserved rural communities in KwaZulu-Natal. Initiated on 13 July 2025, the project conducts monthly clinical and educational outreach sessions at Osindisweni Hospital in Buffelsdraai, approximately 50 km north of Durban.

Through direct patient care, skills transfer to medical staff, and community education, the project works toward reducing cardiovascular disease burden and strengthening the local health system. DHRA remains committed to equitable healthcare access and is actively seeking partnerships to expand specialist involvement and service capacity.

Background and Rationale

Cardiovascular disease (CVD) remains one of the leading causes of morbidity and mortality in South Africa, disproportionately affecting communities with limited access to specialist services. Rural healthcare facilities often face challenges such as:

Shortage of trained cardiology professionals
Delayed diagnosis of cardiac conditions
Limited diagnostic equipment
High burden of unmanaged hypertension and heart failure

Recognising these disparities, DHRA established the Ubuntu Clinical Outreach Project to bring specialist cardiac support directly to rural communities and to build local capacity for long-term disease management.

Project Site

Osindisweni Hospital, Buffelsdraai
Location: ±50 km north of central Durban
Service population: Predominantly rural, low-income community
Key challenges: Limited specialist access, high chronic disease burden

The hospital serves as the central hub for monthly outreach visits and provides a platform for
both patient care and professional training activities.

Project Objectives

1. Improve Access to Cardiac Care

  • Provide regular specialist-led screening, diagnosis, and treatment for patients with
    cardiovascular diseases.
  • Strengthen Local Health System Capacity
Deliver structured training and lectures to medical staff to improve clinical decision making
    and long-term patient management.
  • Raise Community Awareness
Promote early recognition of cardiac symptoms, adherence to treatment, and healthy
    lifestyle practices.
  • Foster Sustainable Partnerships
Engage industry and healthcare partners to scale the project and expand specialist
    involvement.

Activities Undertaken

Monthly Clinical Outreach Visits

Since July 2025, DHRA has conducted monthly on-site visits, offering:

  •  Patient consultations and screenings
  • Management of hypertension, arrhythmias, heart failure, and other cardiac conditions
  • Medication optimisation and referral prioritisation
  • Follow-up care planning to ensure continuity

These visits address both acute and chronic cardiovascular needs, improving patient outcomes and reducing preventable complications.

Capacity-Building for Medical Staff

Each visit includes structured educational sessions for doctors, nurses, and support staff. Training topics have included:

  • Recognition and management of arrhythmias
  • ECG interpretation
  • Hypertensive emergency protocols
  • Evidence-based heart failure management
  • Cardiac risk assessment and prevention

The training focuses on practical, case-based learning to empower clinicians to manage cardiac conditions confidently between specialist visits.

Community Health Awareness Efforts

Although primarily clinic-based, outreach efforts also target the broader community through:

  • Patient counselling during clinic sessions
  • Promotion of early health-seeking behaviour
  • Guidance on risk factor modification (diet, smoking cessation, physical activity)
  • Emphasis on chronic medication adherence

These engagements aim to foster a proactive, informed community with a better understanding of cardiovascular health.

Outcomes to Date

  • Improved access to specialist cardiac evaluation for rural patients
  • Strengthened clinical competencies among Osindisweni Hospital medical staff
  • Early identification of unmanaged or undiagnosed cardiac disease

DURBAN HEART RHYTHM ASSOCIATION
Ubuntu Clinical Outreach Project

  • Enhanced collaborative relationships between DHRA and the hospital team
  • Positive community feedback, reflecting increased awareness of cardiac conditions

While quantitative data will be added as the project progresses, qualitative feedback indicates significant benefits to both patients and clinicians.

Challenges

  • Limited diagnostic equipment at the hospital
  • High patient volumes during outreach days
  • Need for additional specialists to sustain and expand services
  • Resource constraints restricting broader community outreach

Addressing these challenges will require strategic partnerships and additional financial and human resources.

Future Plans and Strategic Partnerships

DHRA aims to expand the Ubuntu Clinical Outreach Project by:

  • Partnering with healthcare industry stakeholders
  • Recruiting additional cardiologists and allied health professionals
  • Enhancing diagnostic capacity through equipment support
  • Increasing outreach to surrounding communities
  • Developing continuous professional development (CPD) accredited training modules

Such partnerships are essential to scale the project sustainably and broaden its impact
